Appointment Information
Appointments and Scheduling
Appointments can be made between the hours of 8:00 am and 5:00 pm Monday through Friday. Please call Phone: 941-625-0984 to make
your appointment. Most patients can get into the office that week or possibly the same day for an appointment in one of our
offices.
Office Hours
Monday - Friday 8:00 am - 5:00 pm
Please bring the following items with you to appointments:
- Your insurance card or other health coverage information. Some insurance carriers require you to have a referral or primary
care physician authorization before treatment is given. Please check with your carrier if you are in doubt.
- X-rays or MRI's not older than twelve or eighteen months (if available).
- Clinic notes or operative reports from your referring physician that relate to your condition.

Workers Compensation
For patients whose injury is work related, we have a dedicated insurance representative to deal with the nuances of worker's compensation, scheduling the initial
appointment with your Workers Compensation Representative, as well as authorization and payment. Please call Kathy at 941-625-0984, Extension 114. Your cooperation
in providing an accurate history and related information is necessary.
Medical Records
If in the future you need to request certain medical records from our office, please be aware that the request could take ten
days to two weeks before you receive the information. We will make every effort to fulfill all medical records requests as quickly
as possible.

Billing Information
We accept Visa, Mastercard, Cash and Check. Co-pays are due upon arrival.
Surgery
If you require surgery, our surgeons will thoroughly explain the operative procedure, the recovery process and the type of
rehabilitation program you will need. Our staff will pre-certify your procedure and discuss any financial implications with you.
After Hours Process
We always have a physician on call in case of after hour emergencies. If you need to contact a physician after hours, call the
regular office number and our answering service will notify the physician on call to contact you.
Routine prescription refills will be filled between the hours of 9:00 AM and 4:00 PM Monday through Thursday and between 9:00 AM and 12:00 PM Friday only. You can make prescription refill requests by calling 941-625-0984 Extension 121.
