Appointments can be made between 8:00 AM and
5:00 PM Monday through Friday. Please call
941.625.0984 to make your appointment. Most
patients can get into the office that week or
possibly the same day for an appointment.

Please bring the following items with you to
appointments:
Your insurance card or other health coverage
information. Some insurance carriers require
you to have a referral or primary care
physician authorization before treatment is
given. Please check with your carrier if you
are in doubt.
X-rays or MRI's within the past twelve months (if available).
Clinic notes or operative reports from your
referring physician that relate to your
condition.

Workers Compensation
For patients whose injury is work related, we
have a dedicated insurance representative to deal
with the nuances of worker's compensation,
scheduling the initial appointment with your
Workers Compensation Representative, as well
as authorization and payment. Please call
941.625.0984, Ext. 105. Your cooperation in
providing an accurate history and related
information is necessary.
Medical Records
The Florida State Board of Medical Examiners
(FSBME) has set limits on fees for copies of
medical records. COC charges $1.00 a page for
the first 25 pages and $0.25 for each additional page thereafter. And a reasonable
fee for the actual cost of mailing, shipping or
delivery. We only release records for visits and
tests done at our facility.

We accept most major health care plans. Please

Billing Information
We accept Visa, Mastercard, Cash and Checks. We are now accepting CareCredit. Co-pays are due upon arrival.
Surgery
If you require surgery, our surgeons will
thoroughly explain the operative
procedure, the recovery process and the
rehabilitation program you will need.
Our staff will pre-certify your procedure
and discuss any financial implications with

After Hours
We always have a physician on call in case
of after hour emergencies. If you need to
contact a physician after hours, call the
regular office number and our answering
service will notify the physician on call to
contact you.
